Form Instructions

Please read these instructions before starting your application.

• The app uses Google Forms. You must sign into Google to fill it out.
  • We strongly recommend signing in with your LakeLocal student email.
  • You can provide a different preferred email address at the bottom of the 1st page.
• To fully complete the application, you must fill out all sections and click Submit on the last page.
• It's OK if you can't fill out the entire application in one sitting. Please be aware of the following:
  • Google saves your progress as you go, but only you can see it.
  • If you need to stop in the middle, simply open the form again and continue later. (Be sure to login with the same google account!)
• You may edit your responses after submitting the form. If you do, be sure to continue to the last page and click the Submit button again. The committee cannot see your changes until you click Submit.
